What are chart notes?
Chart notes are quick, flexible entries you can add to a client’s record at any time. They’re for anything that isn’t a therapy session but still matters for clinical care or administration, such as:
No-shows or cancellations
Collateral contacts
Administrative updates
Clinical observations outside sessions
Any miscellaneous details you want to track
Write and edit chart notes with the same ease as progress notes — format text with headers and bullet points, and even use Smart Edit for AI-assisted adjustments. You can also dictate your chart notes instead of typing.
Adding a chart note
In a client's profile, go to the Chart notes tab.
Click New chart note and choose a type: No-shows, Collateral, Administrative, Consultation, or Other.
Type a title for the note. (You can't save the note without a title.)
When writing your note, you can:
Format the text as needed, and/or use Smart Edit.
If you need to change the chart note's category (Collateral, Administrative etc.), choose from the dropdown menu at the top beside Note type.
When you’re finished, click Add note.
Now, you'll see your note in the client’s Chart notes tab.
Editing and deleting chart notes
You can update or delete any chart note:
To make changes to a note, click on the note in the Chart notes tab, and click Edit note.
Edit as needed, and click Update note to save.
Or, to delete the note, click Delete, and confirm with Yes, delete it.
Downloading a chart note as a PDF
If you need a chart note for documentation or external records:
Open the note you want to export.
Click Download.
A PDF version will download automatically.
Filtering and organizing chart notes
You can filter chart notes to find what you need quickly.
From the Chart notes tab.
Filter by type: Above the list of notes, click No-shows, Collateral, Administrative, etc. to show only that type of chart note.
Sort by date: To sort the notes based on date, click Sort, and choose Newest to oldest or Oldest to newest.
Change notes per page: If you want to see more or fewer notes per page, click the 10 / page dropdown, and choose how may notes to display.
