You can sign progress notes digitally in Upheal, making it quick and easy to prepare your notes for insurance and legal purposes.
Important: Signed notes are locked
When you sign notes, the notes become locked, and you won’t be able to make changes without first unlocking them. If you do unlock a note, you will be asked to provide a reason for unlocking, and your provided reason is recorded in the HIPAA audit log.
Signing notes
To sign a note:
From your client’s page or the sessions tab, click on the session in which you want to sign a note. When you click the session, the progress note is shown on the right side of the screen.
Click the three-dot menu in the top right of the note.
Click Sign note.
To sign, click Sign and lock.
Selecting the check box next to Open appointment payment will open an invoice tab to charge the client for the selected session.
Your note is now signed and locked. If you download the note as a PDF, you’ll see your signature and the date you signed the note at the bottom. If you want to edit your note, you’ll need to unlock it.
Unlocking notes for editing
You can unlock your signed notes for editing. You’ll be asked to provide a reason for unlocking and editing the notes, which will be recorded in the HIPAA audit log.
To unlock a signed note:
From your client’s page or the sessions tab, click on the session in which you want to edit your locked note.
Click the three-dot menu in the top right of the note.
Click Unlock note.
Read the message, and enter a reason for unlocking the note.
Click Accept & Unlock.
You can now make changes, and re-sign your note at any time by clicking Sign note again.
Downloading your notes as a PDF
To download a note as a PDF:
From your client’s page or the sessions tab, click on the session from which you want to download your note.
Click the PDF icon at the top of the note. Your download will start immediately, and the file will be saved to your downloads folder. If you've signed your note, the PDF document will include your digital signature and a timestamp at the end of the note.
