Creating and customizing practice forms with the Upheal form builder

Our in-house clinicians created a helpful library of form templates, including intake forms, consent forms, and clinical assessments, designed to save you time while you build forms that suit your practice.

Templates provided by Upheal are not to be considered legal advice. If you're not sure how to use these templates or how they apply to your practice, consult a qualified legal professional.

A note on template updates

Upheal periodically updates our built-in form templates, to improve language, add new fields, or reflect regulatory changes.

When a template is updated:

  • Your existing forms based on Upheal templates stay exactly as you built them. Your form is a separate copy that you fully own and control.

  • You won't see unexpected changes to forms you've already sent to clients.

  • If you want to incorporate updates from a revised template, you can create a new form and customize it from the updated template.

Creating a form

You can use a customizable template (recommended) or build your own form from scratch.

To create a form:

  1. In the main left menu, go to Forms.

  2. Click New form, and choose Start with a template, or Start from scratch.

    The first time you create a form, click View templates to start from a template. Clicking New form at this time will start you with a blank form.


  3. In the form builder, customize your form to your practice's needs:

    • Enter a Form title and optional description at the top of the screen. Clients will see both when they receive the form.

    • To add a field, click it, or drag it from the left sidebar into the form.

    • To reorder fields, click and drag them in the form.

    • To edit a field, click on it. You can rename the field label, toggle whether it's required, or change its options.

Form elements

Choose from a variety of formatting and field types to create a form that fits your practice. These elements include preset sections, formatting, and a selection of field types, so customizing your form is quick and easy.

Preset sections

Preset sections are pre-grouped fields. Add these sections to make form building faster.

Formatting makes your form easier for clients to read.

Field types

You can add the following fields types to your forms:

Likert scale

Rating scale (see below)

Likert scales

Likert scales are available in the Upheal form builder. A Likert scale presents a statement or question and lets clients rate their response on a numbered or labeled scale. These scales are commonly used for symptom tracking, satisfaction surveys, and clinical assessments.

To add a Likert scale:

  1. In the left sidebar, find Likert scale under Fields.

  2. Click or drag it into your form canvas to add it.

  3. Click the field to edit:

    • Set the scale range. You can set up to five response options.

    • Toggle the Scored switch on if you wish to assign point values to responses, and Upheal will calculate the total score when your client submits the form.

    • Enter your first question or statement in the Ask a question field.

    • Click Add question to add additional questions or statements.

Using variables

Variables are placeholders that automatically fill in specific details, such as your client's name or your practice name. Upheal makes sure the right information appears for every client when you send a form.

To add a variable:

  1. Add a Text block to your form canvas.

  2. Click in the text box to edit it.

  3. To open the variable menu, click Add variable, or type a bracket [.

  4. Select the variable you want to insert — for example, Client name or Practice name.

The variable will appear bolded in a purple box.

When the form is sent to a client, Upheal replaces the variable with the correct information.

Example
If your template says:

"Hi Client name, please review and sign the following consent form for Practice name."
Your client will see:

"Hi Jordan, please review and sign the following consent form for Sunrise Therapy Practice."

Available variables:

  • Provider name

  • Provider email

  • Practice name

  • Practice email

  • Client name

  • Client email

Variables are only available in Text block fields. They are not available inside field labels.

Form categories and labels

To categorize and label your forms:

Click Settings in the top toolbar to open the form settings panel, then:

  • Click the Type dropdown menu to categorize your form as Intake, Consent, Discharge, Clinical assessment, or Other.

  • Add custom labels to your form by typing in the Labels field, then click Create [your label]. Your labels are saved for future use in the Labels dropdown menu.

Previewing your form

You can see exactly how your form will look to clients by clicking Preview in the top toolbar.

From the preview window, click Send to select a client to send the form to, or Close to return to the form builder to make any changes.

Saving and finding your form

When you're finished, click Save in the top toolbar. You'll find your saved forms listed on the Forms page under the Forms tab.
