Upheal's form template library
Our in-house clinicians created a helpful library of form templates, including forms for intake, consent collection, and clinical assessment. These are meant to save you time while you create forms that suit your practice's needs.

Creating a form
In the main left menu, go to Forms.
Click New form, and choose Start with a template, or Start from scratch.
The first time you create a form, click View templates to start from a template. Clicking New form at this time will start you with a blank form.
In the form builder, customize your form to your practice's needs:
Enter or update the name and description, which your clients will see when you send them the form
Click and drag elements to reorder
Drag in elements from the left
Change static text and rename fields
To categorize and add labels to your form, click Settings at the top of the form builder.
Choose what type of form it is: Intake, Consent, Discharge, Scored measure, or Other.
Optionally, add your own labels to the form. If you start typing in the Labels field, you can create a new label by clicking Create "
your label".To see how the form will look to clients, click Preview.
When you're finished, click Save.
You'll find the saved form on the Forms page.
Packets
Packets are groups of forms that you can send to your client all at once. This saves you time when sending forms, and streamlines the filling and signing experience for clients too.
To create a packet:
In the main left menu, go to Forms.
Go to the Packets tab.
Click New packet, and choose Start with a template, or Start from scratch.
Enter or update the packet title and description, which your clients will see.
Click and drag your saved forms into the packet.
When you're finished, click Save. You'll see the saved packet on the Forms page in the Packets tab.
Sending forms and packets
You can send forms and/or packets to your clients, and they can fill them out easily online. Then, you can see the completed forms in Upheal.
Sending forms and packets from the Forms page:
From the Forms page, click the three dot icon beside any form or packet, and click Send.
Select the client/s to whom you'd like to send the forms.
Click Send.
The client/s will receive an email with a link to fill out the forms online. Once they're finished, you can see completed forms in Upheal in your clients' profiles.
Sending forms from the client's profile:
In the client's profile, go to the Forms tab.
Click Send forms or packets.
Choose the Forms or Packets tab, and select what you'd like to send.
Click Send.
The client will receive an email with a link to fill out the forms online. Once they're finished, you can see the completed forms in Upheal in the client's profile.
Forms and packets reminders
You can set up automatic gentle reminders via email if clients haven't filled out their forms yet.
In Settings, go to Client reminders.
Under Form reminders, choose the reminder frequency:
Smart schedule: Reminds the client to fill out their forms 7 days, 3 days, 24 hours, and 4 hours before their next scheduled session, plus weekly reminders if the forms remain incomplete.
If not completed: Sends a reminder at your set interval (daily, weekly, or monthly) until forms are complete.
Before session: Sends one reminder at your set time period before their next scheduled session (1 hour, 24 hours, 48 hours, 1 week) until forms are complete.
After each session: Sends one reminder at your set time period after each session (immediately, 24 hours, 48 hours, 1 week) until forms are complete.
You can set up multiple frequencies of reminder.
When you're finished, click Save.
