When your therapist sends you a form, you can complete and sign it in the Client Portal.
Completing and signing forms
To fill out and sign a form in the mobile app:
Go to the Forms tab in the bottom right.
Select the Incomplete tab.
Forms you haven't filled out yet are listed here. Click Review and sign on the form you want to complete.
To fill out and sign a form on desktop:
Click the Forms tab on the left.
Click Review and sign on the form you want to complete.
Viewing completed forms
To view a completed form in the mobile app:
Go to the Forms tab in the bottom right.
Select the Complete tab.
To view a completed form on desktop:
Click the Forms tab on the left.
Both your complete and incomplete forms are listed here. You can also filter by selecting the Incomplete or Complete tabs at the top.
